My shiatsu will not quit pooping and peeing in my house i have tried taking her out n puppy pads.?

When we come back in she pees or poops. She will use the pe pads overnight now but only because shes locked in the bathroom. What am i doing wong? Help!!!!!!!!!!!!!!!!
